Release of Liability
The sport of water polo is classified as a “physical contact sport” and athlete participation can - and occasionally does - result in athlete injury. Although most injuries are minor, serious and even life-threatening injuries can occur requiring immediate medical emergency services, care, and treatment.
Gold Country Water Polo is a member of United States Water Polo (USWP). Without exception, every registered SWP athlete and coach must also be a member of USWP. All coaches, officials and athletes registered with USWP are covered with general liability insurance.
This is to certify that I, as parent/legal guardian of registered athlete, am over the age of 21. I acknowledge the aforementioned risks associated with the sport of water polo and participation with Gold Country Water Polo to include - but not be limited to - all practices, tournaments, as well as traveling to and from any events associated with Gold Country Water Polo.
Further, I consent to and authorize any adult Gold Country Water Polo coach to seek and obtain medical emergency services, care and treatment for the aforementioned registered Gold Country athlete should the need arise. The coach is to do so in a timely manner and every attempt is to be made to contact me.
Additionally, I, as the parent/legal guardian of registered athlete, agree to be fully responsible for any and all expenses incurred and related to any such medical emergency, care and treatment. I agree to hold Gold CountryWater Polo and all others (such as Ponderosa High School, Union Mine High School and other practice site locations)harmless for such expenses / liability except as provided by the USWP membership of said athlete and that provided under USWP for Gold Country Water Polo membership and / or the athletes personal and private medical insurance providers.
Code of Conduct:
I (we) consent to abide by the below described Rules of Conduct while I (we) are associated with, participate with, or are under the auspices of Gold Country Water Polo. This includes both domestic and international travel.
I (we) understand that violations may result in full or partial forfeitures of a registered athletes or chaperones privileges without recourse and / or other disciplinary proceedings.
1. The transportation, possession or unauthorized use of alcoholic beverages, illegal substances or drugs (except as prescribed by a licensed physician) is prohibited.
2. The use of an ID card by an unauthorized person is prohibited. (Example: Using another individuals USWP membership card, drivers license, passport, etc.)
3. Unacceptable behavior will not be tolerated including - but not limited to - the following: gross misconduct (i.e., inappropriate horseplay, theft, fighting, etc.); blatant disrespectful attitudes, body language or speech, harassment of any kind based on religion, sex, creed, gender, athletic abilities or any act considered to be offensive under federal, state, and local laws. The aforementioned shall be specifically prohibited if directed towards any host facility / directors or if directed towards anyone under the auspices of Sacramento Water Polo, USWP members or officials, parents or guardians.
By signing up for our programs, you acknowledge that you are the parent/legal guardian of the athlete being registered for this event and that you agree to the Release of Liability and Code of Conduct as it appears above.
